Dear Parents: If your child is interested in participating in the Annual Canoe trip from Indian Township to Sipayik, children must be accompanied by their parent or legal guardian. Guardians must sign the release of liability form. Forms must be submitted to Donald Soctomah or Gal Frey PERMISSION SLIP/RELEASE OF LIABILITY FORM By signing this permission slip/release of liability, I, as parent/legal guardian, give permission for my child to participate in the Passamaquoddy annual canoe trip from Indian Township to Sipayik. I, as parent/legal guardian, hereby waive any and all claims for damages of any kind against the Passamaquoddy Tribe, the organizers of the canoe trip or the Indian Day Committee, and further agree to indemnify and hold harmless the Passamaquoddy Tribe and its officers, employees and agents from and against any loss, claim, damage or liabilities that may arise out of, or related to, the Indian Day activities or the Canoe Trip. I waive liability from injuries, faults, and any other emergency situations that may arise during my child’s participation with the Passamaquoddy Annual Canoe Trip from Indian Township to Sipayik and the Passamaquoddy tribe at Pleasant Point and the Passamaquoddy Tribe at Indian Township. By signing I waive all rights, claims and other legal actions.
